Authorization for Release of Information

Path: Index > Student Information > Forms > Module: Health

To create a form for a student, select the student then browse to the Forms tab using the path above. Once there, click the blue New button in the lower left corner.

Scroll down and look for the Health module, or use the search filter at the top. Click on the specific form you wish to open. “Authorization for Release of Information”

To Print the form for the student, click on the Print button at the bottom of the screen as shown below.
