Flagging and Documenting NCH Students

Columbus City Schools has a partnership with Nationwide Children's Hospital.  We currently have 11 Nationwide Children’s Hospital (NCH) primary care sites located in our schools. In addition, we have 24 mobile units at other sites to address students’ health and medical needs.  For students to be treated, or seen by the nurse practitioner, there must be an up to date consent form on file.  In addition to our 35 NCH sites (11 primary care, 24 mobile unit), all consent forms in the district’s 111 educational facilities need to be scanned into Infinite Campus and flagged to document parent consent for medical treatment and NCH’s access to student data. 

How to Upload NCH Consent form to a Student’s Documents Tab

IMPORTANT:  Only upload the first two pages and not the confidential medical information on the third and fourth pages.

  • Click on the Search tab.
  • Select Student from the drop-down menu.
  • Enter the student’s last name in the Search field.  You can also enter a comma after the student’s last name and enter the student’s first name as well. 
  • Click Go.
  • Click on the student’s name in the search results.
  • Click on the Documents tab.  
  • If you do not see Documents, go to Index > Student Information > General
    • If there are currently documents uploaded, they can be seen on the Documents tab.
    • Click Upload New Document
  • A description of the document should be entered in the Comments field, as it is helpful when students have many documents.
  • Click the down arrow to select the Upload Type.
    • Choose NCH: Nationwide Children's Hospital Consent Form
  • Click Upload File.
    • Find the file to be uploaded and click Open.
    • You will return to the Documents tab.
    • Click Save at the top of the tab.
    • The document can be viewed on the Documents tab.



Adding a Flag For Nationwide Children's Hospital

Please Note

It is important to flag only those forms where the “yes” box on the second page was checked by the parent in order to give CCS to share individualized information to any partner. We will give information out only when parents give specific permission to do so. The flag is not to identify that the form is uploaded, but that the parent gave permission to share information.


Path: Index > Student Information > General

  • Click the Search and select Student from the drop-down menu.
  • Type the student name, then click Go.
  • Click on the student’s Flags tab.
  • Select the New icon.

                       

   

  • The Student Flag Detail editor will appear on the screen. Select the Nationwide Children's Hospital from the drop list.

  • Enter the Start Date (use the date the parent signed the form)
  • Click the Save icon when finished.  The flag will display next to the student’s name the next time you search for them. 

Make sure to end date the flag with the 20th of August the following year (8/20/2xxx), as this is an annual consent form, but requests for data are often made over the summer.  This will automatically remove the flag from the main screen in Campus after this date.  For example, if you are setting up flags for the 17-18 school year, you would end date with 8/20/2018.
