The Unable to Locate Student Checklist formally known as the 15 Day Checklist, is used to document attempts to locate a student prior to withdrawing the student to Code 74: Unable to Locate Student. In order to submit an End Date Withdrawal request, this form must be completed, signed by the principal and uploaded to the student's document tab in Infinite Campus. Please note this process, and the form is a collective effort to locate students who have displayed a consecutive non-attendance pattern of full-day absences.
