Submitting a Withdrawal Request - CCSDAS
The Columbus City Schools Distributed Application Services (CCSDAS) is a collection of custom-built applications representing a significant advance in process management. The CCSDAS Withdrawal/End Date Request application will serve as a replacement for Perfect Forms.

The withdrawal request is then submitted to the principal for approval. Once approved by the principal it is submitted to Central Enrollment for final approval and processing. |
