A student's Cumulative (CUM) record should follow the student from school to school and district to district. If a student has enrolled at your school, and you have not received their cumulative record, you will want to submit a Records Request to the previous school(s).
Columbus City Schools has a standard Records Request Form to be used. Click on the file to download to a word document. The school name, address, fax and phone number can be edited to fit your schools information.

Requesting Records from external schools/districts
When a student is enrolling in your school from a school outside of CCS, an Infinite Campus Fillable Record Request form should be generated within 24 business hours. The request should be sent to the previous school and recorded on the Records Transfer Log in IC.

Fulfilling Record Requests through Scribbles
Scribbles K-12 Record Requests for students enrolled in your school on the student information system should be processed and withdrawn within 3 business days. A withdraw should be submitted in CCSDAS utilizing the Scribbles K-12 Record Request form (unless the request specifies it is for informational purposes only).
Scribbles K-12 Record Requests for students who are not currently enrolled in your school on the student information system (Infinite Campus) should be moved to the CCS school of enrollment as reflected in Infinite Campus. If the student never attended ("no show") the school where the student information system shows enrollment, the cumulative folder should be returned to the last CCS school of attendance once the withdrawal has been submitted.
