1. Select a student, click the Student Miscellaneous Tab.

2. Parent/Student Acknowledgement Form (Page 24): A check mark on Signed Code of Conduct indicates this form has been signed and returned.

3. Acceptable Use Policy (page 25): make the appropriate selection.

4. Consent for Release of Student Information Form (Page 26)

   1. Directory Information  use the drop-down menu for applicable selection.

   2. Photographs, Audio, Video, or Electronic Images use the drop-down menu for applicable selection.

   3. Original Works of Art and Written Materials use the drop-down menu for applicable selection.

   4. Quoted Statements use the drop-down menu for applicable selection.

   5. Written Letters use the drop-down menu for applicable selection.

5. Parent-Student-School Compact Form (Page 27) - A check mark on Parent-Student-School Compact indicates this form has been signed and returned.

6. Census Verification Form (Census>Reports>Census Verification): if distributed to parents, check the box if returned.

7. Click Save.
