When entering information, you'll note the category and sub-category are listed. You can change them here if you need to.

The Tell Us More About It field is for the specific details of your issue. Please enter as much detail as you can.
Enter the best phone number to reach you during the day. This can be an office/desk number or a cell number, whichever you prefer.
Screenshots are VERY helpful when working on many issues. If you have a screenshot, or another attachment that will be helpful in solving your issue, you can attach it by clicking the Browse button or dragging a file from our computer into the box.
Once you have completed entering all fields and adding any attachments, click Send.
Email Confirmation
You will receive an email stating your form was created and will include the Request (Ticket) number assigned to your request.
Track the Status of your Request
In the upper right corner of the home screen, there is a button labeled "Requests" with a number indicating how many open requests you have
