CCS is responsible for providing 1095C form to employees each year by the end of March. This statement is similar to a W2, except it is for health coverage. It provides the IRS with the months of coverage for employees and dependents. See example below. Once these files have been processed, we also send an electronic file of coverage directly to the IRS.
