Submitting a Withdrawal Request - CCSDAS

The Columbus City Schools Distributed Application Services (CCSDAS) is a collection of custom-built applications representing a significant advance in process management.  The CCSDAS Withdrawal/End Date Request application will serve as a replacement for Perfect Forms. 

To Access CCSDAS

  • Click here to access the DAS Withdrawal Platform for processing a withdrawal.

Directions for submitting a withdrawal request in CCSDAS

 

  • Enter Username

  • Enter Password

  • Click the Sign in button

 

From the navigation panel on the left side:

  • Expand Student Information

  • Expand Withdraw

  • Select Create

 

  • Enter Student #

  • Select the Student

  • The School - will automatically populate based on the student’s enrollment.

  • Enter Withdraw Date - Please note this should always be the day before their start of their next school and can be a nonschool day.

  • Summer Withdrawal - Check this box if the student withdrawal is before the start of the school year or did not return from summer. The withdrawal date for a summer withdrawal should be 07/01/20XX.

  • Withdraw Reason - Select the appropriate reason from the dropdown menu.

    • Please refer to the Columbus City Schools End Status Withdrawal Codes

    • Commonly used withdraw codes and the IRN to withdraw to are listed below

  • Select Withdraw To IRN from the dropdown menu

  • Next School - Enter the name of the next school

  • Next Address - Enter the address of the next school

  • Next School CIty - Enter the city of the next school

  • Next School State - Select the state of the next school from the dropdown menu

  • Next Zip Code - Enter the zip code of the next school

  • Supervisor Name - will automatically populate based on the student’s enrollment

  • Supervisor Email - will automatically populate based on the student’s enrollment

  • Comment - Enter any relevant comments for the withdrawal request

  • Attach documentation - Attached supporting documentation for the withdrawal

    • Chose File - Select the file

 

Withdrawal applies to this Enrollment in Infinite Campus

This information is automatically populated based on the student’s enrollment and this will be the enrollment that the student is withdrawn from

 

Please Note:

  • Withdrawals can only take place after the form has been completed and the form is approved by the building principal.

  • Please place CUM folder in your inactive student records for Student Name.

  • If the student is enrolling in another CCS school, please forward the CUM folder to the enrolling CCS school upon written request.

  • Supporting documentation MUST be scanned and attached to this form.

 

 

  • Click on the Submit button to submit the request.

The withdrawal request is then submitted to the principal for approval. Once approved by the principal it is submitted to Central Enrollment for final approval and processing.
