This document is provided for parent to review at time of receipt. Do NOT collect a signed paper version of this document. The parent will be asked to verbally acknowledge that they understand the obligations outlined within the agreement form.
Additionally, the parent will receive an electronic document on Campus Parent to sign digitally with in 24 hours of receipt of the device.
