Requesting a Transfer of Records

Once approval from The Division of the Registrar is received, you will send boxes to Digitization at Central Enrollment. To have the boxes sent to Central Enrollment you will need to complete a “transfer of records” request. Please follow the steps below to insure a smooth process.

 

  1. Go to the CCS Intranet (click HERE for direct link)

  2. Click on “Departments”

     

  3. Select Purchasing, Delivery Services, Print Services, and Warehousing

     

  4. Select Delivery Services Work Request (DSWR)

     

  5. Once selecting DSWR, a small box will appear at the bottom of the screen. It is the downloaded document.  This is what it will look like:

     

  6. When this document opens, click on the Enable Content button in order to make the appropriate selection.

     

  7. Select the appropriate drop down from:“Click Here to Select Service Type From Drop Down List”

     

  8. Select “Transfer” from the drop down menu:

     

  9. After selecting “Transfer”, click on gray box to proceed

     

  10. Complete the Transfer form. Records are to be sent to garage.

     

  11. Save a copy of the Transfer Request sheet as a PDF and email it to DeliveryServices@columbus.k12.oh.us with that same request number in the subject line

  12. Print a copy of the PDF and place it with the box(es) that is/are being transferred.

  13. Please make a notation on each box, stating the school’s name, or the original location of those records if the school was a closed building.
