A student's Cumulative (CUM) record should follow the student from school to school and district to within the Columbus City School district.  If a student has enrolled at your school,  and you have not received their cumulative recordrecords, you will want to should submit a Records Request to the previous school(s). Columbus City Schools has a standard Records Request Form to be used. Click on the file below to download to a word WORD document.  The school name, address, fax and phone number can be edited to fit your schools information. Inactive cumulative records shall be maintained at the building where the student last physically attended. The purpose behind this process isschool information.

  • CCS Student moving from CCS school to CCS school
    • CUM folder should be sent when records are requested
  • Students enrolling in CCS from other districts or withdrawing to other districts
    • CUM folder remains at last CCS school of attendance and copies of student records are sent upon request by the new school of enrollment

The CUM folder should always be retained at the last Columbus City School of attendance. The purpose for this process; when a former student is requesting records/requests their transcripts they will remember the last school they attended instead of , not where they were suppose to go expected to attend / or enrolled for the following year.
